




Description: * Bachelor’s degree in Social Work; * Active and regular registration with the Regional Council of Social Work (CRESS); * Availability to work 30 hours per week (150 hours per month); * Prior experience in clinical patient care; * Experience working with patients undergoing renal treatment is desirable; * Knowledge of the National Institute of Social Security (INSS) process is desirable. * Periodic follow-up with patients and their families; * Interview and advise patients regarding access to free transportation and the required documentation, as well as obtaining high-cost medication; * Provide general assistance to families, including identifying patients who miss appointments; * Refer patients with physical disabilities to appropriate agencies; * Facilitate dialysis slots for patients in transit and/or transfer; * Collect and analyze the necessary documentation to initiate patient treatment. 2512220202491228928
