Job Summary: Carry out social work activities, providing support to patients and their families regarding social, economic, and cultural aspects related to the health-illness process. Key Highlights: 1. Psychosocial support to patients and their families at various stages of the disease. 2. Facilitate communication between the healthcare team and family members. 3. Assess and discuss cases with the multidisciplinary team for treatment planning. Carry out social work activities, providing support to patients and their families regarding social, economic, and cultural aspects related to the health\-illness process, proposing measures that contribute to the implementation of medical treatment. **Responsibilities and Duties** * Provide social services to patients of the Institution, identifying and analyzing material, emotional, psychological, or other types of issues, applying fundamental social work methods to assist in patient recovery and promote reintegration into social, family, and occupational environments; * Diagnose and develop intervention plans addressing the social and emotional aspects of the healthcare team/patient relationship through bedside social interviews, aiming to provide psychosocial support and continue case follow-up; * Maintain the emotional stability of the patient and/or family through support, modifying attitudes and seeking solutions; * Develop an annual plan outlining actions and initiatives to be implemented, designed, and/or proposed; * Facilitate communication between the healthcare team and patients’ families, enabling contact between both parties for clarification of diagnoses and planned treatments; * Assist patients throughout various stages of the disease (hospitalization, convalescence, and community reintegration); * Counsel patients’ families upon discharge, death, or other events through individual or group interviews, providing psychosocial support; * Attend to patients dependent on oxygen therapy and home hospitalization by interviewing\-them and their family members, guiding them toward available resources offering such services, and referring them accordingly, aiming to improve quality of life and prevent rehospitalization; * Attend to victims of abuse, conducting interviews with patients and family members, maintaining records, and submitting reports to the Juvenile and Family Court, Police Station’s Child Protection Unit, Elderly Council, CRAS (Centro de Referência de Assistência Social), and Social Promotion agencies; * Attend to women victims of sexual abuse and other forms of violence, providing psychosocial support to patients and their families; * Attend to indigent patients by contacting municipal shelters to secure vacancies and arranging transportation post-hospital discharge; * Request ambulance transportation for patients from their municipality of residence in discharge cases; * Accompany family members during patient transfer from Adult and Pediatric ICUs, preparing reports and respective internal referrals; * Assess and discuss cases with the multidisciplinary team to gain comprehensive understanding of the diagnosis and proposed treatment. **Requirements and Qualifications** * Bachelor’s degree in Social Work; * Active CRESS registration; * Prior experience in the field is desirable. **SPDM** \- Associação Paulista para o Desenvolvimento da Medicina is a non-profit, philanthropic civil association, recognized as being of federal, state, and municipal public utility, respectively by Decrees No. 57\.925, dated 03/04/1966; No. 40\.103, dated 05/17/1962; and No. 8\.911, dated 07/30/1970\.
