Provide clear and comprehensible information about the disease, treatment options, medications, tests, and self-care, empowering patients to make informed decisions and manage their own health. Coordinate and facilitate access to various health services (consultations, tests, procedures, therapies), ensuring continuity of care across levels of care (primary, secondary, tertiary) and with the multidisciplinary team. Identify and assist in overcoming obstacles preventing patients from receiving necessary care, such as transportation, financial, social, cultural, or communication difficulties. Conduct ongoing follow-up of beneficiaries, monitoring their adherence to the care plan, clinical progression, and overall well-being, implementing interventions when required. Active participation in health programs: Collaborate in the development, implementation, evaluation, and monitoring of health programs, continuously improving care navigation and coordination processes. Requirements: **Education:** Bachelor’s degree in Nursing with active registration at Coren. Postgraduate degree in Case Management, Public Health, Primary Health Care, or related fields. A Master’s degree in Nursing, Collective Health, or related areas is desirable. **Requirements:** Knowledge of Supplementary Health (ANS), depending on scope of practice. Knowledge of case management, care coordination, primary health care, and health programs. Familiarity with health care networks and their workflows. Knowledge of chronic health conditions (e.g., diabetes, hypertension, cardiovascular diseases, cancer) and their respective care plans. Knowledge of health education and patient empowerment strategies. Experience in preparing managerial reports and analyzing health data. Intermediate proficiency in office application suites (with emphasis on word processing, spreadsheets, and presentation tools). Relevant work experience.
