Job Summary: A nursing professional to provide continuous and rationalized care to the patient portfolio, performing consultations, procedures, and care coordination. Key Highlights: 1. Perform systematic nursing consultations and procedures. 2. Coordinate care for the beneficiary portfolio and manage patient flow. 3. Participate in case discussions and strengthen patient relationships. Provide assistance to the patient portfolio, responding to demand continuously and rationally by conducting nursing consultations, requesting complementary exams according to team planning and legal provisions governing the profession (SAE), with documentation in the electronic medical record system. **Responsibilities and Duties** * Conduct systematic consultations (SAE) documented in the electronic medical record, requesting complementary exams per protocols and legal requirements. * Provide direct care in clinical emergency and urgent situations, ensuring appropriate referral for continuity of care. * Conduct home visits and perform technical procedures within the nurse’s scope of practice, such as wound dressings, preventive exams, and other prescribed interventions. * Carry out epidemiological and sanitary surveillance across all life stages, strengthening trust-based relationships with patients and their families. * Coordinate and navigate care for the beneficiary portfolio, ensuring smooth patient journey flow. * Ensure patient access to higher-complexity treatments or hospitalizations, managing flow between different levels of care. * Integrate actions with occupational health services to avoid duplication of care and optimize beneficiaries’ health indicators. * Participate in case discussion meetings and assistive model reviews, jointly establishing action plans with the medical team. * Compile and conduct critical analyses of portfolio health indicators, generating technical reports to support decision-making. * Periodically monitor equipment functionality, sanitary conditions, and service resolution capacity. * Provide data for audits, maintaining up-to-date physical or electronic documents and records per quality management standards. * Analyze process failures, propose solutions to ensure work continuity, and manage internal and external conflicts. * Organize and coordinate follow-up groups for specific pathologies and health education activities. * Stay updated on beneficiaries’ family contexts, promoting risk prevention actions and reinforcing the right to health. * Strictly comply with occupational safety regulations, using PPEs/collective protective equipment and adhering to ANS and professional regulatory body guidelines. * Ensure ethical confidentiality and safeguard sensitive information concerning patients and the company. * Perform other duties inherent to the position, as required by the immediate supervisor and based on technical qualifications. **Requirements and Qualifications** Bachelor’s degree in Nursing with active registration at COREN.
