We currently have an open opportunity for a Clinical Pharmacy position. If you have prior experience and identify with this role, please submit your application and grow with us. * Issue technical opinions on materials and medications; * Stay updated on legislation and regulations applicable to pharmacy services, disseminate necessary information, and ensure compliance; * Evaluate requests for non-standardized medications and, where possible, substitute them with standardized alternatives; * Collaborate to ensure all procedures performed and the department’s environment comply with the Hospital Infection Control Center (NCIH); Control and issue APACs (Authorization for High-Complexity Procedures) for outpatient patients according to the manual; * Manage inventory of central and/or satellite pharmacies; conduct stock assessments and, when necessary, place purchase orders for materials and medications with critical stock levels to prevent shortages; * Conduct pharmacovigilance activities regarding used medications; * Develop staff schedules, duty assignments, and organize leave, shift swaps, as requested by management; * Draft and coordinate implementation of standards and routines; * Design, deliver, and encourage team participation in educational activities aimed at professional development and improvement; * Advise and alert physicians and other healthcare professionals about potential errors in prescription and use of medications and materials, documenting interventions in patients’ medical records; Participate in the hospital’s multidisciplinary teams as directed by management; * Attend courses and training sessions as instructed by management; * Provide patient care within the department, delivering pharmaceutical care; * Coordinate with other hospitals, if needed, for borrowing medications and materials, recording such transactions in the system and ensuring full official documentation, including signatures and stamps; * Monitor antimicrobial use across all hospital departments jointly with the NCIH; Prepare and supervise dilution and fractionation of medications, disinfectants, and other products per the manual; * Receive suppliers and evaluate new products potentially suitable for hospital use. * Issue technical opinions on materials and medications; Record significant events occurring during daily activities and important notices for the next shift or day in the incident logbook; * Select and/or oversee training of pharmacy employees, interns, and pharmacy students; * Supervise dispensing of psychotropic, narcotic, and similar medications strictly according to medical prescriptions, controlling and documenting this process; Supervise your work unit to ensure physical space, furniture, and equipment remain clean, organized, and operational, issuing service orders to responsible departments for repairs when necessary; * Supervise and control expiration dates and integrity of medications and materials to prevent expiry and ensure product quality. If disposal is required, verify adherence to proper disposal procedures per the manual; * Supervise and carry out product dispensing according to medical prescriptions; * Resolve pending issues in the hospital information system (MV) related to patient care; * Participate in and support management in the biannual revision of the Pharmacotherapy Manual, conducting required pharmacological and pharmacoeconomic studies; * Perform material and medication counts for inventory reconciliation; * Assess established quality indicators and propose new ones; Supervise activities and conduct of pharmacy assistants; Supervise returns of materials and medications; * Supervise and control release of high-cost materials and medications; * Review and sign daily entries in the pharmacy logbooks; * Document patient consultations in medical records, including pain management.
