Job Summary: The Health Program Monitor on the Viver Bem team will actively engage with clients enrolled in the Chronic Conditions Care Pathway, monitoring and supporting treatment continuity. This opportunity for a **Health Program Monitor** is to join our **Viver Bem** team — the department responsible for promoting comprehensive care and encouraging well-being and quality of life among our clients. This professional will specifically operate within the **Chronic Conditions Care Pathway**, primarily engaging with our clients proactively via digital channels and telephone, monitoring and supporting effective and uninterrupted treatment progression. Essential traits for this role include empathetic listening, strong verbal and written communication skills, resilience, and proactivity in achieving performance goals. **Responsibilities and Duties** * Conduct virtual and home visits, or visits at Unimed’s own facilities, alongside patient monitoring; * Monitor the clinical assistance process; * Promote routines aligned with quality and patient safety culture; * Audit and critically assess processes with a focus on continuous improvement; * Monitor patients with chronic diseases and/or other pathologies according to risk factors defined by care pathways, via telephone contact; * Record interventions and patient progress in designated systems/platforms; * Support implementation of action plans and care plans for projects approved by the National Health Agency (ANS), aligned with the life course pathway; * Generate and analyze program indicators, proposing improvements and action plans; * Manage individualized care plans for patients participating in care pathways; * Monitor/evaluate activities conducted by third-party companies serving cooperative clients. **Requirements and Qualifications** **Education for this position:** * Completed Nursing Technician qualification. **Qualifications:** * Familiarity with Office suite (especially Excel), and systems such as Totvs, CRM, SAE HealthMap, and Blip (WhatsApp). **Additional Information** **We are a team passionate about caring for people — and we do it purposefully, delivering excellence with passion and sparkle in our eyes!** We are ranked the **2nd best health company in the GPTW ranking** and the **5th best company to work for in Rio Grande do Sul (RS)**! **And more:** This year, we achieved the **45th position in the GPTW Brazil Ranking!** All of this reflects an outstanding workplace climate, built by people who believe in what they do! **Join us in achieving even greater success!** PROUD TO BE GPTW RS, BRAZIL AND HEALTH!
