




Job Summary: A Nursing professional to coordinate patient assessments, care plans, budgeting, and implementation within a post-acute healthcare network. Key Highlights: 1. Responsibilities and Duties: * Receive evaluation requests from health insurers and forward them to the evaluating nurse to ensure completion within the shortest possible timeframe; * Monitor response time for evaluations conducted in hospital or home settings; * Conduct case discussions to define the appropriate Home Care Treatment Plan (PAD) for each case; * Ensure submission of budgets to health insurers (OPS) within contractual guidelines and within 24 hours; * Follow up on submitted budgets; * Analyze rejected budgets; * Guarantee accurate entry of information into the IW system; * Coordinate with dehospitalization departments; * Maintain positive relationships with health insurers (OPS); * Submit preliminary implementation documentation and follow up accordingly; * Provide all necessary resources available to ensure safe patient implementation; * Transcribe physician orders (PM) and nursing prescriptions (PE) for hospital discharge; * Ensure patient implementation at home, adhering to company-established deadlines; * Monitor implemented patients for 48 hours; * Hand over shift responsibilities for implemented patients to the case manager;" Prerequisites and Qualifications: * Completed Bachelor’s degree in Nursing, with active COREN registration; * Postgraduate qualification in auditing or management is desirable; * Minimum 5 years’ experience in Home Care, Transition Clinics, or ILPIs (Institutions for Long-Term Care of the Elderly); * Operational knowledge of the IW system is desirable.
