Promote health education and prevention activities among residents within the assigned area. Record information and data collected during visits for follow-up and activity planning. Coordinate with health and social assistance teams for referrals and support to served families. Previous experience as a Community Health Agent, Community Facilitator, Community Educator, or Social Mobilizer may indicate suitability for some of the responsibilities of this position. High school diploma required.
