




Job Summary: Coordinate and supervise matrixing actions, interdisciplinary care services, and cognitive stimulation workshops for the elderly, as well as provide support to caregivers. Develop strategies for welcoming and supporting family caregivers ***Responsibilities and Duties*** * Coordinate matrixing actions with Primary Health Care teams, promoting training and technical support for caring for elderly individuals with dementia; * Organize and monitor individual interdisciplinary care services, in collaboration with professionals such as psychologists, nurses, and nutritionists; * Plan and supervise cognitive stimulation and rehabilitation workshops for the elderly; * Develop strategies for welcoming and supporting family caregivers, including discussion groups, guidance sessions, and psychological follow-up; * Monitor indicators, service flows, and outcomes of implemented actions; * Coordinate with the health network and other services to ensure continuity of care; * Conduct meetings with the Itajaí Municipal Social Assistance Secretariat, the Municipal Council for the Rights of the Elderly, and targeted meetings with AMFRI and other public agencies. ***Requirements*** * Completed undergraduate degree in a health-related field; * Active registration with the respective professional council; * Leadership profile, organizational skills, and strong communication abilities; * Proficiency in computer applications (Word, Excel, and Canva). ***Univali values inclusion and equal opportunity; all positions are open to persons with disabilities and INSS-rehabilitated individuals.*** Availability to work Monday through Friday, from 8:00 AM to 2:00 PM Compensation: R$4.500,00 - R$4.900,00 per month Benefits: * Medical assistance * Dental assistance * Education allowance * Partnership agreements and commercial discounts * Private pension plan Workplace: On-site
