A Nursing Professional responsible for patient triage, appointment scheduling, telemonitoring, care coordination, and direct patient care, with a focus on health promotion and education.

**Description and Responsibilities:**
**Working Hours:** 8:00 AM to 5:58 PM

**Employment Type:** Permanent – CLT
* Perform patient triage for medical appointments at CAS;
* Schedule consultations and procedures according to program service routines;
* Conduct patient telemonitoring, including admission, follow-up, and satisfaction evaluation regarding the program;
* Coordinate care for patients participating in programs;
* Provide direct health assistance to patients at CAS or at home, performing low-complexity nursing procedures to support the medical team, addressing service demands such as medication administration, wound dressings, ablation, among others, per medical prescription and technical knowledge;
* Measure vital signs;
* Encourage greater participation of patients and their families in the proposed treatment, promoting health education;
* Assist the multidisciplinary team in procedures, when necessary;
* Record all activities in accordance with established protocols in the electronic medical record;
* Identify critical cases among beneficiaries and coordinate with department management;
* Improve beneficiaries’ quality of life and act in prevention and health promotion;
* Promote Preventive Medicine initiatives among colleagues;
* Minimize difficulties in adherence to the offered therapeutic plan;
* Conduct health profiling for beneficiaries;
* Collect laboratory specimens;
* Monitor temperature and humidity levels in the laboratory/refrigerators;
* Register and order supplies via system;
* Track lot numbers and expiration dates of laboratory materials and products;
* Register exams in proprietary system;
* Administer medications and immunobiologicals;
* Provide patient assistance, ensuring comfort and well-being, while adhering to good practices, safety norms, and procedures;
* Maintain cleanliness and organization of the triage room, outpatient room, and consultation rooms, restocking supplies and medications per checklist;
* Clean and disinfect medical-hospital equipment per Institutional Protocol;
* Wash, clean, and disinfect equipment in the decontamination area per institutional schedule;
* Test daily all work equipment (defibrillator, cardiac monitor, pulse oximeter, etc.);
* Regularly inspect medications and solutions used in consultation rooms and outpatient areas, properly discarding expired items;
* Inspect daily the emergency cart seal and contents, recording in the control protocol;
* Perform other related duties consistent with the nature of the role, as directed by superiors.
**Requirements:**
* Completed Technical Nursing Education;
* Active COREN registration;
* Basic knowledge of Primary Health Care;
* Basic knowledge of Microsoft Office Suite;
* Proficiency in medication administration;
* Proficiency in venipuncture;
* Proficiency in wound dressings.
