**Description and Responsibilities:**
**Schedule:** Monday to Friday, 8 a.m. to 5 p.m.; Saturdays, 8 a.m. to 12 p.m.

**Employment Type:** Permanent – CLT
* Perform pharmacy activities, including organization and cleaning of the department, organizing medication shelves and refrigerators, as well as dispensing medications;
* Supervise, control, and execute activities related to medication restocking to maintain regulatory stock levels within the institution;
* Monitor biweekly verification of emergency medication carts;
* Conduct inspections in CAS departments to verify proper storage of medications, large-volume parenteral solutions, and preparation of medications for administration;
* Promote awareness among multidisciplinary teams regarding care practices, storage, and importance of rational medication use through informational bulletins and lectures;
* Perform clinical pharmacy services;
* Conduct psychotropic drug inventory reconciliation;
* Ensure triple-checking of materials occurs;
* Provide pharmaceutical counseling and guidance to beneficiaries;
* Monitor various implemented projects, including group sessions or home-based care;
* Conduct individual consultations focused on the protocol established for each project;
* Proactively identify patients requiring pharmaceutical care;
* Serve as Technical Responsible (TR) for the department, as needed;
* Provide customer service via telephone or electronic channels;
* Guide and allocate staff optimally so that essential services—such as prescription dispensing and chemotherapy preparation—are not compromised during atypical situations like staff shortages or absences, thereby avoiding patient harm;
* Keep staff informed about institutional requirements through meetings or bulletins;
* Identify departmental needs and propose continuing education initiatives for teams;
* Provide information to all multidisciplinary team members regarding medications and pharmacy routines;
* Make responsible decisions in urgent situations;
* Organize refresher courses for the Pharmacy team and other multidisciplinary teams involved in medication handling;
* Evaluate and ensure implementation and review of clinical protocols and Standard Operating Procedures (SOPs) related to services provided by the Hospital Pharmacy Department;
* Procure materials after analyzing medical requests and usage indications;
* Record all activities in accordance with established protocols in the electronic health record;
* Monitor program indicators linked to this position;
* Participate in projects as organizer, speaker, or advisor, when required;
* Perform other related duties consistent with the responsibilities of their operational unit and nature of work, as directed by superiors.
**Requirements:**
* Completed undergraduate degree and active registration with the professional council;
* Knowledge of Clinical Pharmacy;
* Skill in Humanized Care;
* Knowledge of Health Promotion;
* Knowledge of Risk and Disease Prevention;
* Skill in Assertive Communication;
* Proficiency in Microsoft Office suite.
