




Job Summary: Nursing intern to support patient admission processes for home-based care, focusing on organization, analysis, and quality improvement of care under supervision. Key Highlights: 1. Collaboration in clinical case discussions and protocols Work under supervision to support patient admission processes for home-based care, contributing to the organization, analysis, and quality improvement of care, in alignment with the academic scope of the Nursing internship. * Conduct **data collection during initial patient and/or family assessment**, following institutional guidelines, with emphasis on identifying needs, clarifying information, and proper documentation * Support the **analysis of adverse events, non-conformities, and process failures** related to admission, contributing to information gathering, data organization, and technical and operational improvement suggestions, under the supervision of the responsible nurse. * Contribute to **facilitating communication** among health insurance providers, patients, families, and the care team by relaying information, providing initial guidance, and documenting according to institutional workflows and protocols. * Support the **assessment of home-based care feasibility**, assisting in collecting and organizing information regarding the patient’s and family’s clinical, social, and environmental aspects. * Participate in the **patient transition process from the hospital setting to home-based care**, assisting in monitoring admission workflow stages and contributing to care fluidity and continuity. * Collaborate in **clinical case discussions**, with an academic and observational focus, as well as supporting the development, review, and organization of institutional protocols and workflows—particularly those related to the admission process. * Assist in **collecting, organizing, and analyzing admission process indicators**, contributing to information generation for continuous improvement and supporting team decision-making.
