Together, let's build the greatest healthcare community the world has ever seen! **Responsibilities and duties** * Provide periodic support to patients and their families; * Interview and guide patients regarding access to free transportation and related documentation, as well as obtaining high-cost medications; * Provide general assistance to families, identifying absent or non-compliant patients; * Refer patients with physical disabilities to specific agencies; * Coordinate dialysis slots for patients in transit or transfer; * Collect and analyze necessary documentation for patient treatment initiation. **Requirements and qualifications** * Bachelor’s degree in Social Services; * Active and up-to-date CRESS registration; * Availability to work 30 hours per week (150 hours monthly); * Previous experience in clinical support; * Experience working with renal treatment patients is desirable; * Knowledge of INSS procedures is desirable. **Additional information** **What we offer?** * Health Plan: to take care of your well-being and your family.
